Suicide Risk

Screening vs.

Assessment

Suicide risk screening is a very rapid way to identify someone who needs further assessment for suicide risk.

Suicide risk assessment is an evaluative conversation with the patient that further triages suicide risk, assesses risk and protective factors, & guide next steps.

The suicide question on the Patient Health Questionnaire-9 (PHQ-9) is NOT an evidence based screening tool. Relying solely on this question leads to underidentification of individuals at risk, missing over 30% of potential cases.

Suicide Risk

Safety planning is an effective evidence-based technique to reduce suicidal risk. Alongside your patient and their guardian, identify effective coping techniques and supports to use in a crisis or when suicidal thoughts emerge. Avoid using "no-suicide contracts," which are not considered best practice.

Safety

Planning

Safety planning is a collaborative conversation, NOT a solo worksheet project.
