The suicide question on the Patient Health Questionnaire-9 (PHQ-9) is NOT an evidence based screening tool. Relying solely on this question leads to underidentification of individuals at risk, missing over 30% of potential cases.

Safety planning is an effective evidence-based technique to reduce suicidal risk. Alongside your patient and their guardian, identify effective coping techniques and supports to use in a crisis or when suicidal thoughts emerge. Avoid using "no-suicide contracts," which are not considered best practice.
